Our Services

Healthcare Management

Healthcare management is the effective administration and coordination of healthcare services and resources to ensure the provision of high-quality care and support to individuals in need. As a healthcare manager within the NDIS framework, our role is to promote collaboration among various stakeholders, including healthcare professionals, service providers, and participants, to achieve optimal outcomes and enhance the overall healthcare experience for individuals with disabilities.

EFA seeks to provide and assist with strategic planning, organisation, and oversight of services aimed at managing and improving the well-being and quality of life of people within the interabled and aging community.

Coordination Support

Coordination support is the assistance provided to individuals with disabilities to navigate and access the supports and services available to them. Coordinators act as a central point of contact, working closely with participants and their families to understand their goals, needs, and preferences. They provide personalized guidance, information, and assistance in developing and implementing an individualized plan that aligns with the participant's aspirations and circumstances.

We aim to enhance your ability to exercise choice and control over your life while effectively managing your NDIS plan and taking part in a vital role to ensure that you receive the necessary assistance, resources, and opportunities to thrive and achieve you desired outcomes within the NDIS framework.

Task Administration and Management

Task administration is systematic management and oversight of the various tasks involved in delivering supports and services to individuals with disabilities. This encompasses the coordination, monitoring, and evaluation of tasks related to participant plans, funding allocation, service agreements, and service delivery.

Task management refers to the structured and coordinated approach to planning, organizing, and overseeing the tasks involved in delivering supports and services to individuals with disabilities. It involves the efficient allocation of resources, tracking progress, and ensuring the timely completion of tasks related to participant plans, funding, service agreements, and service delivery.

EFA seeks to implement robust task management and administration practices and policies while adhering to the NDIS framework to enhance the overall service delivery experience, maximize participant outcomes, and foster a person-centered approach to interabled and aging support.

Community Engagement Management

Community Engagement Management is a strategic approach of actively involving and collaborating with various stakeholders to promote inclusive practices, gather feedback, and ensure the effective implementation of interabled and aging supports and services. It involves establishing meaningful relationships between you and families, carers, service providers, community organisations, and advocacy groups.

As a community engagement manager, we facilitate dialogue, disseminate information, and seek input from the community you are interacting with to better shape policies, programs, and decision-making processes that foster relationships, promote awareness, and address any concerns or barriers that may arise.

Health-Based Mentorship

A health-based mentor is a knowledgeable and experienced individual who provides guidance, support, and expertise in healthcare-related matters to individuals with conditions that impact their well-being. Mentors provide guidance, advice, and practical assistance to participants in navigating the healthcare system, managing their health conditions, and achieving their health-related goals. Health-Based mentors work closely with participants, providing personalized support tailored to their specific needs and circumstances.

EFA offers its knowledge and insights on healthcare practices, facilitate access to appropriate healthcare professionals and services, and empower you to take an active role in managing your own health and well-being. By engaging us in Health-Based mentorship, you can gain valuable skills, confidence, and independence in managing your health, leading to improved overall health outcomes and a higher quality of life within the NDIS framework.

Self-advocacy support is assistance and guidance provided to people to empower them to express their needs, make informed decisions, and advocate for themselves effectively. It is the provision of resources, strategies, and assistance aimed at empowering individuals with disabilities to become active and confident advocates for their own needs and rights. It involves fostering self-awareness, developing communication skills, and providing tools for individuals to articulate their goals, preferences, and concerns within the NDIS framework.

We support you to navigate the complexities of the system, understand your entitlements, and help you actively engage in decision-making processes related to your NDIS plans and services. We provide training, peer support networks, and individual coaching, all geared towards enhancing self-determination and promoting meaningful participation in shaping one's own life journey.
